this has happened to me twice now, zoning into a mythic dungeon. just gone from my bags.
on second thought this might be a product of the auto loot power ups.
First time happened between looting a WQ rare and accepting a summon to Eye of Azura.
Second time happened in Arcway either when i zoned in or as i sat dusting my old loot waiting to start the M+.
