We’ve developed a fix for this issue, and it was implemented with scheduled realm restarts on April 23 at 7:00 a.m. PDT.
When realms come back online, we expect the Vault to generate offerings with more variety, as intended.

We’ve identified an issue in 11.1.5 that can cause the Great Vault to rarely offer the same item in multiple reward slots. For example, a player who completed eight Dungeons last week could potentially see the same trinket offered twice in that row.
We’re investigating the issue, but it is extremely unlikely that any fix will be retroactive to this week’s rewards. Blizzard Customer Support cannot assist with rerolling or modifying generated Vault rewards in any way.
