Last two days, multiple times and multiple groups have had Kharon not be able to be spawned. No red book. We’ve tried every combination of route, not going in library before Kharon, doing opera first vs giant skeleton, killing all 3 big aboms, etc. It just randomly bugs out and is not summonable. Pissing me the heck off. Fix your game.

It’s because it’s Karazhan Crypts not Kharon Crypts.
every time where ive gone top floor → opera->spider/skele->lich i havent had an issue.
One time we skipped a boss, or dropped down the well, and never could spawn the book
I have had the book not spawn on 1 of 7 runs. All 7 runs took the same path. It was bugged on run #4.
It was around 2-3am server time on the morning that they hotfixed the relics to make them more obvious.
Dark Rider chambers - banshee - opera - flame bone guy - spider - Karen
Maybe the book is triggered by making sure you get the Crypt Keepers. Maybe the bosses. Maybe a bug.
Guarantee my route is working currently though 
I’ve never not had the book spawn. We always clear all the trash and kill all the other bosses and then the book spawns.

I’ve had the book not spawn twice. Once on the normal Casters > Opera > Harbinger > Spider > Kharon route
Another time doing the reverse route.
Both times, it’s the Casters altar that isn’t lit up, and we can see one of the casters still alive in their cell despite having released and killed one.
Fix this Blizzard. It’s cost me 2 out of 5 Kharons.
Same issue here, and as others describe seems to be random. Same path, same # of mobs killed, went back and cleared the instance after it didn’t spawn, no change. Help!
You need to kill the 3 Crypt Dweller, for the red book so spown!

My group hit this bug for a 2nd time and we believe we’ve found what is causing it. We freed Barian Maryla, and Sairuh Maryla did not die as scripted. Instead she was sitting and standing in a corner over and over, even after we killed Barian.
We later noticed in the room for Kharon there are 3 altars that correspond to the Harbinger of Sin, Creeping Malison, and the Apprentices, you can tell by the trappings near them. The altars Malison and Sin were both lit up, the one for apprentices was not.
We will not be picking Barian going forward, as we had no issues with Kharon spawning, after picking Kaigy to kill.
