Dreadwyrm crown has been lotted twice from mythic Nighthold and has not been added to my appearances at least twice now. It is in the bag of my druid now but will not unlock the appearance.

That is because this item is a Death Knight tier item. Currently looted tier pieces cannot by learned other than by their specific class. Tier tokens can be learned by anyone.
Next patch they will be changing this and all transmog can be learned by any class. If you keep the item in your bag, it will automatically get added when this goes live.
