Much like N’zoth, the mantid, etc. she really doesn’t need them. The body she hijacked is flesh, but she just floats everywhere so it’s not like she risks damaging herself.

So many people fixated on her feet.
I think the number of people actively discussing her feet is a little stranger than the fact that she doesn’t wear shoes.

Shoes are for to protect feet from the ground.
Xal’atath floats, her feet don’t need protection from the ground.

It’s not just her, this has been going on for a while. Someone on the WoW dev team, likely several of them, have a serious foot fetish. Look at the entire Nightborne. Djaradin. Vulpera. Individuals are all over the place, there are legit a lot of them.
