Can anyone confirm whether or not, as of Dragonflight, Bloodsail and Steamwheel’s reputations must be met at the same time in order to achieve “Insane in the Membrane”? I’m seeing a lot of outdated stuff, some saying that players must have the reputations at the same time, and others that as long as you’ve had once once, its okay to lose it for the other one.
Does anyone know the answer?

To earn this achievement, You’ll need to have all of the Factions required for it at Exalted. Plus you’ll need to have Bloodsail at Honored as well at the same time to earn the achievement.
My suggestion is to grind your Bloodsail reputation first. I did this and grinded them to Exalted before I worked on my Steamwheedle Cartel reps. You can grind reputation for all of the Streamwheedle Cartel reps without losing Bloodsail rep by killing pirates in Tanaris!
Nope, you don’t need to have those two rep at the same time. Get bloodsail rep to exalted first then ignore bloodsail completely and focus on the rest. I was hated by bloodsail when I got my “Insane” title this year, I don’t remember the month.

I had went down to Hated with Steamwheedle by time I got my Bloodsail rep up to Honored and finished the achieve.
I am still Hated with them – really need to work on that – hard to travel anywhere right now, lol.

Good to know as the requirements must have changed after I earned this particular achievement. When I got it completed you had to have everything done at the same time to earn it.
we earned our titles very close together… there was no need to have the reps at the appropriate level at the same time.
it was over 10 years ago, so wonky memory on the subject is forgiven
