They should have a healing spec that uses runic power and blood to heal others.

They aren’t evil paladins.
That being said… I love dark themed healers like that so short of getting a necromancer class with a healer spec that’s the next best thing.
I really dig necromancer healing in ESO.

But DKs use the power of Death to pull blood out of things, and to empower their own blood. I don’t think it works in reverse.
Imagine a warrior trying to squeeze the juice back into an orange they just crushed.
We arent evil. We were taken over and condemned to be death knights for eternity by the light king. 
They aren’t even the opposite of paladins. They are Death, the opposite of Life. Life is not a creation of the Light.
Void is the opposite of the Light, and we have no pure void class. Shadow Priests are the closest thing, but they aren’t really the counterpart to Paladins either.
A new Void Knight class would be amazing though, but I don’t have high hopes for it ever being a thing.

Could imagine the health hazards? what if the person whose blood they were using to heal you with had a disease…

They used to be able to heal their minions with death coil.

More like when a Druid overheals you, you have to rake up the extra leaves.
If a Blood DK overheals you, you explode like a balloon filled with salsa.

I don’t necessarily believe that. Demon Hunters worked in Legion because of Illidan returning.
Right now, necromancers seem pretty bad, but with the way they’re setting up this cosmology war anything is possible. The light clearly not being concerned with being “good” for example.
Death can make a turn just like the Light did, Yrel being a perfect example.
In the end I think when they said Sylvanus is “morally gray” it was really misplaced and should just be said about all of the forces of the universe, it’s all what the intentions are and how it’s applied.
Necromancers could have a redemption arc (lol ew) and be introduced after Shadowlands after showing their intentions of not being entirely evil in Shadowlands.
That said I HIGHLY doubt they’ll ever be a thing because Death Knights already exist.
They wouldn’t have a path available

You’re probably thinking of the Anti-paladin of D&D fame.
Death knights need an anti-healing spec. Drain the life of your allies to heal yourself.

and other undead provided themselves if they used the lich ability…
