I feel like im behind on content, like a whole heap.
I gather im like possibly 3-6 months behind on content mostly due to life events keep taking over and not getting much self time for myself to just play wow.
With next patch hitting shortly, and me just catching up to where im trying to get to undermine while doing the whole ADHD mission thing where im somewhere doing something, How far will i be behind after this patch hits?
Im just bascially struggling hard

Don’t worry! Being behind is fine, this is not some job where someone is depending on you to finish this content already.
It’s probably better to be a little behind because you always have something to do, rather than waiting around for new stuff.

You can basically just do the dailys and catch up super fast, along with weeklys for heroic gear, then theres also the coins or what ever you can get heroic gear by doing LFR

Thats my raiding life pretty much, Just LFR, Heroics and Delves at this stage
There is still an 11.2 patch to come and it will have a sort of soft reset in item levels and catchup. So you can just wait until then to catch up. It will probably be available in 8ish weeks following their previous releases and the roadmap.
Stop worrying about keeping up with the Joneses and play the content at your own pace. It isn’t going anywhere.

You can never really be that far behind, by design.
This expansion has insane catch-up mechanics.
Like, for PvP, if you hit 1400 rating, you can start buying these relatively cheap boxes containing the best PvP gear, just send them to alts and bam- they are as geared as your main.
PvE is a bit trickier but there’s Catalyst, Delves, M+, and now those Undermine raid token thingies for Heroic gear. (And you can do LFR for some free heroic pieces from that quest.)
As for rep? They always increase it the further along we go into the expac.
The good news is every time a new patch drops they end up turning out more catch up mechanics that make it fly three times faster. This is why I never do content when it’s current. I simply get to it when I can get to it. It’s just a game and I’m not in competition with anybody not even myself.

I mean it’s literally a garbage heap, so there’s that.
It took me 2 months from hitting 80 to get where I am today and you’re coming back at a 640 ilevel so realistically less time than me if you’re willing to do M+ and delves, let alone raiding.
Here’s some basics:
Focus on one toon to start. If this is your main, stick with it. Do farming on alts if you need gold, but don’t try to level and gear a bunch of alts otherwise you’ll never “catch up”.
Your ilvl is pretty decent for jumping back in. Go do LFR and clear it each week. This will give you valorstones and crests you can use to upgrade what you have.
Jump into Mythics. Start in the basement, work on your 2s. Get a feel for the dungeons. Work your way up while you get some gear and learn the dungeon paths.
Find a Normal raid. There should be a bunch listed in Group Finder. Everyone will be fighting for gear but you won’t lose every roll. This will get you more valorstones and crests for more upgrades.
Work your way up the Mythic “ladder” until you get to 10s. Focus on trying to time those. Once you have them down and have all the portals, you’ll be set.
Don’t worry about the story right now. It’s not THAT important at this stage. When the next story patch hits, if you follow the steps above you should be ready and “caught up” for the next raid/dungeon cycle.

Never mind. It’s just a game. I’m in a similar situation. Haven’t played at all for 3 months since I crashed my car and I’m staying with my folks to recover. I’ve accepted that im just going to miss half this expansion. Although that did become less acceptable the other day when i say they’re doing recolours of the new tier 2 sets, and they’re only available short term! Booooo!

Sympathies, it must be frustrating. I hope that you are up to date with the campaign stuff and the storyline, that is the most important. There are a lot of catchup mechanics and, well, sometimes things get left out or behind. Skipping the Undermine zone isn’t a terrible thing to happen.
There isn’t a whole lot to be behind on. Once you’re at max level you can get geared up to hero track ilvl pretty fast, and from there you can do whatever you want. Want to clear normal or even get AOTC? No problem at this point in the season, it should be a breeze even if you just jump into a pug group. Want to work your way up to KSM or KSL? No problem, just takes doing.
Unless you’re dead set on progressing through the entire mythic raid for CE, you’re not too late for anything. In fact, you’ve got more options and can achieve more things in less time than you could have at the start of the season.
It’s also not super important to interact with any more than you want to. Once you finish the campaign, which doesn’t take too long, you can just ignore it without any ill effects.
Just focus on one part at a time and make use of skips where they’re available.
If you mean behind in gear, focus on getting gilded crests and using your catch up sparks to make 681 ilvl gear.
If you mean on the story/campaign you can do it in a matter of days I’d think. Reps and other things are all optional and about cosmetics mostly now.
Easiest way to get the gear is to do the flame radiance failed. The Nightfall quest runs every hour and the three dailies can be done relatively quickly. All will give you gear.
As for content most new features can be completed in about an hour. After that it is just a process of repeating to get reputation. Note, this excludes Visions and Duo battles as neither feature was something I wanted to do so I skipped them.
Truthfully my best advice is just break it down into smaller steps and work your own pace.
