How do you use this ability effectively? I feel like it does 0 damage and it bugs and breaks you out of stealth and puts you in combat when it expires.

It taking you out of stealth is dumb but its rare you re stealth that soon after doing sepsis. Its damage and mastery increase is pretty sweet though.
I can never find a good time to press it, usually I want to SS the opener for the crits, by the time I have an opportunity to use sepsis the go is over.
Maybe I can use it right before a go in 3s for the mastery, I forgot it gave us that.
I’ve only played RBGs since I went Night Fae, but I’ve just throw it in where ER was in our Kyrian opener for the mastery buffs for big eviscerates. I don’t know that its most efficient tbh though. So coming out of stealth with everything off CD:
Cold Blood > Cheap Shot > Dance/Symbols/Badge/Blades macro > Sepsis > Evis > MfD > Evis
It’s been a really heavy hitter if you can get it all off. If Sepsis isn’t dispelled then the ticks add up on top of the other damage.
If I’m really going all in, I Vanish offensively then Cheap Shot > SS > Evis to keep taking advantage of the mastery buff.
Again, can’t really speak to how this would work in arena, but wanted to offer what I’ve been doing.

It gives a lot of mastery if u play niya
I usually CS>shadowstrike>sepsis>evis>CS>ss>cs>evis
It will buff evis like 20 and also gives 12% health
