In his 30s maybe. Like he looks like he is in his 40s now. Assuming that he was relatively close in age to his brother and his brother kept aging for the decade or so that had passed, I feel like that is a safe bet. Someone may know a more accurate number though.
Lore answer? Around the same time Sylvanas did. I believe he died to the scourge. (30s?)

30s was original body he is currently using the body of his nephew.

I though it was his brother’s body. Hmm, well that would change some stuff then.
Yeah, tired of hearing “HEH, speaker of the Horde…” too.
It was his much younger cousin. It showed him in the story ‘Dark Mirror’ as a little boy (checking the story he was 9) when living Sylvanas came by to canoodle with living Nathanos (who was probably in his 30s when Rammstein the gorger killed him) >_> It was pre-Legion when they captured said cousin and used him as raw materials to ‘refresh’ Nathanos’ standard Forsaken model into his new intact one, so assume the cousin as a grown man serving the Argent Crusade was probably 20s/30s too.
