How is item level determined

I finally got the horseman’s helm, at level 89, and the item level was 112. (Wow Head shows it at 116??)

If I was of higher level, would the item level be higher? Does all loot scale based on a players current level? So running an instance with people 20+ levels higher, does the loot still scale to me?

Loot will scale to your level. Other people in the instance won’t have an effect.

Wowhead having a different ilvl listed for your level is probably just a glitch in Wowhead.
