How exactly does this item work? Do i have to activate it?
You have to have the Honorable Medallion achievement in order to use it. Enter a BG or Arena and it'll unlock automatically.
To use the item itself, drag the icon from the Honor tab to your action bar to use it like a spell. It'll be active whenever you're in a PvP engagement, whether you're out in the world in War Mode, in a BG or in an Arena.

also it will not work in open world unless you have pvp active. if you want it in open world you'll need to get the heirloom trinket.
