I know that ICC 10 and 25 man share lockout, so when doing the achievements for “glory of the…” I had to do 10 man 1 week and 25 man the 2nd week.
Now I’m doing Hellfire Citadel, saw that one of the bosses (Socrethar) is just too difficult on Mythic, so I ran out and changed it to 10 man normal. It reset the whole raid and now I have to go and kill all the bosses again.

Regarding difficulty, do ALL raids have Normal, Heroic and Mythic levels? I don’t think so, but how can you tell? (same goes for dungeons)
SOO introduced flex mode. any raid after that is flex unless mythic which is 20-man set, so the 10/25 for WoD and beyond is a non-issue. At that point, normal, heroic and Mythic all have their own lockouts.
There is no in-game indication whether a raid shares lockouts across difficulties, and you won’t know unless you look online or discover the shared lockout yourself in-game. This is information that should be communicated in game, but it isn’t for some reason.
Ideally, every difficulty would have its own lockout, but we unfortunately don’t live in an ideal world. We live in a world where Blizzard makes inconsistent decisions which contradict their other decisions.
I agree it’s a bit annoying trying to figure out the pattern of these things. Although previous to SOO, you can run Firelands normal and heroic from Cataclysm in the same week. I only know because I ran it a lot for the Dragonwrath legendary questline.
This only became a thing when firelands became a timewalking raid. Prior to that Firelands shared the same lockout rules as all raids from Naxx / OS / EoE up to and including Throne of Thunder.
Every raid from Naxx / OS / EoE / VoA up to and including Throne of Thunder excluding Ulduar and Firelands (you can run normal and heroic firelands in the same week).
No. Mythic difficulty started in the WoD prepatch and to test out the difficulty, blizzard applied it to Siege of Orgrimmar (final raid of MoP). So any raid from SoO onwards has mythic difficulty. Normal, heroic and Mythic for those raids do not share the same lockout. LFR difficulty does not share the same lockout. So you could do LFR Dragon Soul and 25 man Dragon Soul.
The whole 10 / 25 split in terms of achievements and item appearances only apply to Wrath era raids (minus Ulduar due to timewalking where blizzard combined the loot tables and achievements). So there is no reason to run Mogu’shan Vaults on 10 man these days. You are better off doing it on 25 man (get more loot that way). Going back to the wrath era raids, originally 10 man, 25 man and their heroic counterparts (ToC and ICC only) all had separate lockouts. However when Cata rolled around, blizzard made them ALL share the same lockout due to the changes they made to 10 / 25 man in Cata (i.e. they now reward the same gear and item level) while in Wrath, 10 man rewarded lower quality gear than 25 man.
With respect to dungeons, heroic mode applies to TBC dungeons onwards with some vanilla dungeons getting a heroic mode due to revamps over the years. These include;
Mythic difficulty for dungeons started with WoD in patch 6.2.5. So only dungeons from WoD onwards has mythic difficulty and they have a weekly lockout. You can do heroic and mythic modes of dungeons as they do not share a lockout. Normal mode dungeons can be repeated over and over again (you need to reset all instances prior to running them again) however there is a 10 instances (dungeons and raids) per hour limit. That applies to your account. It is important to note that the following dungeons do not have a normal mode;
