I did the Stormwind Visions last week and I got the Black Blood to drop from doing the side areas. I’m doing Orgimmar this week, doing the side areas, saving the Shaman, etc., except I cannot get the Black Blood Coagulate to drop? I’ve done it like 5x, nothing. What am I missing?

Welcome back adventurer. I have made a new discovery but I need to test it out. Hidden in the Lost areas of the Horrific Visions I believe is a rare black blood coagulate.
I read the quest multiple times. What does Lost Areas mean? I am not PVE’er, I never did these quests before. It was not clear as to where to go. For the Stormwind Visions any side area gave you the needed items. But thanks for chiming in after the fact, very helpful.
They’re documented in the map, in the tooltips right in the game.
Admittedly, Blizzard doesn’t always document things, but they actually did here. Short of beaming the information directly into your brain, there isn’t much else they could have done.
I watched a streamer struggle today with the same issue. Once again thanks for nothing, some people just look to make their candle brighter by blowing out the candle of others. This community is so elitist and toxic.

As mentioned before, that’s because there are only two areas in the zone that award the quest item: both of the “Lost” areas (Valley of Honor and Valley of Wisdom in Orgrimmar).

Two is, by design, the max you can get in one run.
