The ‘hold and release’ for all empower spells on all 3 specs is not functioning correctly.
Think is happening to more than half of casts.
Would hold to the ticks wanted, let go and it doesn’t get casted.
Sometimes would work if you combined it as if it were both ‘hold and release’ and also tapping it at the wanted tick, but 'hold and release is selected in the options.
this happens to me specifically because i made a macro for tip the scales and had to take it off my bar because it was doing what you describe.
If i cast it normally with no macro it works. Takes slightly more brain power when i want to use tip the scales

I had a macro with tip and trinket
Game Menu > Options > Accessibility > General > Empowered Spell Output > Change to ‘Press and Tap’
Why this isn’t the default option, I don’t know.
It is a problem that happens with the macros, for some reason it seems that it overwrites/deactivates the function of hold and release.
I use macros alot and it’s been the main detractor of the class for me. Empowered spells in a macro are just absolutely terrible and usually don’t even cast unless you use finesse that defeats the purpose of having the macro.
This is the setting I use too, works so much better for me.
This bug was fixed at some point then came back.
Removing macro fixed it for a bit, but still janky particularly with fire breath
