Do you use a gaming keyboard? Or a regular one?
I once had issues pressing many buttons at once because my keyboard could only handle having 3 keys pressed at once.
This went away when I bought a Razer gaming keyboard.
Just thinking… Ctrl-Alt-1 + a movement key could cause one of the other keys to not function as expected.
As someone who plays Druids and Shamans, I don’t use any 3x key bindings. I find using tilde to 5, with F and R are enough, with them having a ctrl, shift and alt variant each.
Non combat things can go on other, less comfortable binds.
That’s 32 keybinds all within comfortable reach of my left hand.
The mouse buttons on my Master MX 3 are rebound to these modifiers:
—The gesture is bound to ctrl+alt, and the top one is bound to modifying combo: ctrl+shift+alt.
The top one is ctrl+shift+alt with 4 casts nova, which is shift+4.
and the side one: ctlr+alt with 4 casts portal, which is ctrl+4.
Also, if I remove the mouse from the equation and use my keyboard, the same events happen. I am using layer 2 on UseVia and PowerToys, but nothing is remapping Lalt, Lctrl, or LShift.

My solution was to just bind ctrl+shift+4 to something that way ctrl+4 does not accidentally trigger.
But I have so many things to learn, omg. This expansion was really bad. I am using Supernova 123 and Ice Nova 123; introducing Supernova brought a new playstyle, but also I have to have 2x frostbolts, 2x flurries, and 2x ice lances to remove the excess frost, excess fire, and frostfire empowerment because it cleaves and breaks CC. I also only play Supernova as spellslinger frost anyway. Like why, lol.
Swear to god, this expansion added like 10 binds for me, and I am not even trolling. I still have not picked up db+ring+wall+ring of fire playstyle, which is standard FFB play, because I do not have the binds or experience with it, but do have experience with ring of frost+db; however, I do not agree with the mechanics. I believe a mage should never be in melee range unless forced to. Moving towards the healer to setup db+ring feels bad to me and is not how the game should be designed. I wish they would just put some people in charge who knew about the game. Oh well.
