Outside of the Mage Tower, on target dummies and regular mobs Swipe and Thrash work correctly. However, inside the Mage Tower, the buttons don’t cause the skills to fire. Issue is intermittent, where they will work for one attempt, and then not all all for subsequent attempts.

Do not cross a loading screen, such as entering the Mage Tower, while in form. If you happen to do this anyway, shift out of and back into form and the abilities will start working again.
This is a new issue with 10.0.5.
