I’ve been having this issue since Pre-patch launched where on Guardian Druid only, whenever I shift into Bear form I get a random version of it. I thought this was a glitch with the game, but earlier in a dungeon I saw a Guardian Druid that wasn’t having this problem. So I’m starting to think it’s a problem with my game itself.
(Note: this also happened to me while testing the pre-patch on PTR)
You probably have glyph of ursol chameleon on; check for a mark on your bear form spell in your spellbook while playing guardian.
You can remove glyphs with vanishing powder, typically sold by reagent vendors/inscription vendors. The inscription trainer in boralus sells it, for example.
