I am not 100% sure, but I feel fairly confident that the Ascended Webfriars’ Grimweave Orb ability present in some delves can bypass Divine Shield’s immunity. My Details’s death recap says I cast Divine Shield 1 second before I died, which was followed by a Nerubian Threadmancer’s Web Bolt (which was immune’d) and an Ascended Webfriar’s Grimweave Orb for ~800k damage (which was not immune’d, and did kill me).
Grimweave orb has this flag, so it’s 100% bypassing divine shield.

Well, that explains it then. Feels weird, and I don’t like it, but if it is intentional… 
then they should fix the tool tip for divine shield or the recap because mine just said the same thing without any mention of passing through an invulnerability. Either way one needs to be updated.
Taking damage while divine shield up is a mention that it’s passing through an invulnerability. I don’t know if its actually intentional for it to have that flag, just explaining that’s why it bypasses divine shield.
