I had the same issue, although mine was just generating 584 gear even after completing numerous lvl 7 and lvl 8 delves. The week i ran the delves, the great vault showed i would get item lvl 616 gear in the delve slot. The next week when i opened the great vault, it only generated an item lvl 584 piece of gear. I opened a ticket to see if the issue could be resolved and was told by blizzard that its something you have to hope people in the forums will figure out. Seems like a terrible response to an issue like this, but thats blizzard for ya. This happened on 6 of my alts whereas the first toon i logged in on was able to get the appropriate reward.

Ran into this myself, was confused since my first two slots were 616 but the other was still 587. Don’t know if it was just a coincidence or what, but I noticed I had a “bonus objective” in my quest log (it was one of the Pact Rumors) and after completing that, I did just a regular Tier 8 delve and the Vault upgraded to 616. Again, could have been a coincidence and I just had to do one more, but just kind of odd, you know?
Oh boy, this behavior is still happening. But what makes me pissed off the most is the lack of fix and even a simple explanation/answer.
