Just ran M Razsageth for this, activated it immediately (but before the cinematic at the end triggered) and it didn’t work and doesn’t appear to have. It unlocked the normal version of the Mage 1800 red helm on the condition I unlock the original (on my Shaman I will add) and no other models have appeared in the appearance/transmog gallery.
TL;DR: This item appears to be currently broken and it persists through multiple relogs.
Please fix.

It only works on tier items, so if you loot a lookalike non class specific shoulder, you won’t get the additional appearances. If you use a shoulder tier token to unlock n/h/m shoulder for that class, you’ll see the second appearance unlocked.
It also can take an hour or two to “register” at least it did for me last season. Eventually I logged in and had the extra appearances unlocked.
Yeah was hoping it’d magically come good and it still could.
I am using the Primalist tokens converted to set pieces and then upgraded using Untapped Forbidden Knowledge so that should count?
Just hope it doesn’t take them months to fix this (like Ele shaman artifact shield) or years (Sepulcher crossbow).
