In Bastion, in the Shadowlands, The Orb of Judgement is not spawning, which takes you to the real Incense of Judgement, meaning you cannot get the Proof of Wisdom which allows the chest to be unlocked, so the chest is impossible to open and the achievement not obtainable

I am having the same issue.
Attempted to reset by doing the following:
Hearthing to another zone.
Killing my character.
Re-reading the scroll at the top.
Order to attempt to spawn Orb:
Patience Bell → Knowledge Incense Bell → Insight Incense Bell → Judgement Platform… Missing Transport Orb to “real” Judgement Bell
My toon is a 9/9 chapter Kyrian story Monk.

having the same issue. I’ve click on the scroll then followed through with patience > knowledge > insight and no orb. Been trying to reset between as well. Unable to complete treasure.
Having the same issue today. Attempted all the strategies listed to no effect.
Same, been trying two days in a row. Orb for the real Judgement is not spawning. Tried before server restart on Monday, and again after restart, today.
WowHead link comments said it is working again. I confmred just a bit ago too.
