Kept mine as a devoted chosen of N’zoth. Not sure why he wanted to hide himself in Xal’atah. Hopefully the bring him back in Warthin or midnight.
At some point mine was converted to the toy and the buff was taken away from the character that had been holding on to it. I don’t remember precisely when, just that it was now missing.
For clarity I had left one of my warriors as a servant and never completed the ‘get rid of this eye’ quest line.
I might be wrong, but if I remember correctly you get the toy if you turn in the quest. You can only turn in the quest if you remove the gift.

One day I logged in to my druid and the buff was just gone & I got a toy in the mailbox.
I gave mine up and kept the toy. But it would have been nice to have something cool. Like others said, it might happen in Midnight. The void is gonna be on the attack and I feel like the old gods just fooled us into thinking we beat them.
They got rid of it afaik, but people who did it during BfA got the Sliver of N’zoth as a toy.
Kind of a shame, because you can do all sorts of PvP shenanigans with that toy.
Hunters, for example, can tame beasts of their own faction by using that toy.
It’s been long enough that I no longer recall enough details to be specific, but I remember a vague feeling of getting the impression that people were reading into the Gift of Nzoth thing more than they were intended to and that it was already “concluded” in Blizzard’s mind.

I can totally at the very least see Xal’atath commenting about it and be more receptive or cautious (maybe both), to both the Legion Priest and anyone keeping the eye.
