Not sure how - but on the second round of doing orbs, my group just “lost” a yellow orb.
No one had it, nor did it exist on the floor anywhere. Had to wipe and reset. Bricked the key in the process. gg

I had it happen twice. After it bugged on a +16, it bugged again on the 15. Did you have a Kyrian in your group? I have a clip of it, and it looks like it disappeared when he used a Kyrian Pot.
