The set bonus from the Frozen Shadoweave set seems to cause threat. For example, the tank in our group will shoot enemy number 1 from a group of 5 enemies. When they close the gap, the tank will start building threat on enemy 1, but not any of the others for now. I hit enemy 1 with one shadow spell, causing the set bonus from the Frozen Shadoweave armor to heal me slightly. Enemies 2-5 all switch targets to me instead of the tank. This doesn’t seem intentional, as similar effects do not generate threat.

The very first comment on wowhead from 2008 says it causes threat exactly like you describe:

It does generate threat. Some times i have gotten aggro in pulls with several mobs where the tank only damages one target.
It’s the same thing as the warrior diamond flask in classic. All the healing pulled threat
