On my demon hunter I have the option for the fel tattoo to be purple, red etc.
Isn’t fel always green or is there lore for non green?

The tattoos are arcane in nature, if memory serves, meant to suppress fel. I don’t think there’s a lore explanation for the color choices; I’ve always written it off as a purely aesthetic thing.
