I've tried using the regular failure detection pylon, and the Reaves in failure detection pylon mode, and it never works. I've even tried dying directly under it with no one in group in combat and it doesn't work. Anyone have any success with it?
This was also the case for our raid. Tried it on a couple Xavius attempts and it never rezzed anybody. Guessing it's either completely broken, or broken on certain bosses.

Can I ask if you guys know for sure if someone who died was right next to the pylon?
I've used it in groups I've been in and it's worked, but only for people who are quite close to the pylon. I'm curious if it's just a proximity thing or if it's actually buggy.

Were they dead before you popped the pylon? If not, definitely sounds buggy for sure.
It's bugged, pulled out a pylon in Kara today, died right on top of it, no rezzes for me or anyone. Have yet to see one work, at all, in raid or h dungeon or mythic, on any boss, this was the 15th wasted !@#$ing pylon. Want to request refund from Blizz for mats.

At leadt you have it. The quest is apparently bugged for me as I cannot get the first part. The NPC is not there

But yes, old res items seem locked to their expansion.
The res item from Tanaan Jungle, for example, does not res characters above level 100. It will *say* it resses them, and give them the res buff, but does not actually do anything.
