I have repaired the files, deleted cache blah blah, all the Blizzard fixes, including installing newer drivers.
I use no add-ons AT ALL. I use the Default UI too.
It’s been happening in dungeons only. This started on 8.1.5 and stopped for a bit, then started happening again. It’s annoying for someone that does a lot of keystones.

I’ve seen a lot of recent 132 crashes being caused by overlay programs running in the background. If you’re using Discord’s overlay, try disabling it in that program’s settings. You may also want to just try closing other background programs.
We’ve also see that changing your video mode to Dx11 or Dx11 Legacy can prevent some of these crashes.
I just wanted to second this post. I updated everything, reset UI, and disabled all addons (like you.) Still get lagged / disconnected / action bars switched almost everytime I run a Mythic+.
It’s frustrating enough when I’m by myself, but now I’m letting other people down on these runs.
No programs in the background, including Discord. I’ll try turning back to Dx11, see if that helps.
It’s been a day since I disabled the overlay, and chosen Direct X11 Legacy it seems to have fixed.
I am still gonna keep testing just to make sure
