I used the Enchanted Wrym’s Dream Crest in Creating a Weapon. The enchanted dream crest gives a level 476 item, but I ended up with only a 463 item. It was a personal order of the highest quality, and i used only high quality items in creating it.

Double-check that the crest was applied to your weapon. You can drop the weapon into a recraft request at the commissioner to see what’s in it.

Is the weapon full-rank? I know you mentioned high-quality reagents and that you said ‘personal order of the highest quality’, but if the weapon didn’t get the crest applied then this is the alternative.
Guilty of this particular snafu myself. Crest stayed safely in my bags lookin pretty.
