Groups in dungeons get wiped out. Is there any one particular type of player or class that is to blame for the wipe most of the time? Is there a constant behavior that can be pointed to? Is there perhaps a class that contributes to dungeon wipes the most? I really don’t know. Just curious what the consensus is?

This is the ONLY answer based off your question-- because it’s just too vague.
There are ABSOLUTELY situations where the tank failed. The tank IS to blame.
There are ABSOLUTELY situations where the healer failed. The healer IS to blame.
There are ABSOLUTELY situations where the DPS failed. The DPS IS to blame.
But as to which is which?? It all depends on the circumstances and variables involved.
When in doubt, blame the hunter.

Open details and look at deaths and damage taken. Usually will tell you.

Even then that’s not always a good indicator. For instance if people die because the healer couldn’t keep up with unavoidable damage then that would be the healer’s fault despite it not clearly saying so in the logs.
The tried and true tradition of any player worth their salt will ALWAYS blame the healer. I’m a healer, and I blame myself for wipes. Dps pulls other mobs? Obviously the healer couldn’t keep up. Tank didn’t use proper defensives? Obviously the healer couldn’t keep them alive.
Just think like this and you’ll go places.

Not always. On higher keys especially (depending on the affix) some trash packs and some bosses can pulse out quite a bit of unavoidable damage all on their own without pulling extra. If the healer isn’t prepared for these damage spikes with a cooldown then people will die.
I’ve also seen it where the tank needed better gear and took damage more easily than might be expected.
The real answer is EVERYONE or NOBODY.
It’s a group effort. They succeed or fail as a group. Wipes happen, but if the cause isn’t communicated and fixed, that’s a group failure. Surely, someone in the group knows what went wrong and how to fix it. Look for solutions instead of blame.
Once or twice or even three time sure try again but fourth or fifth than you start wondering who is making the mistakes.
