So, in a somewhat seriousness tone, Blizzard owes me a pair of tier gloves. 
So, just ran a PuG Normal VoI tonight, which went “well”… and we killed Dathea, and she dropped 2 Mystic Garnet Forgestone’s.. and well.. this very lucky person… whom I marked out there name on purpose… won both tokens. Even thought the “Need before Greed” system said that it couldn’t happen…
You can even see that I could have one the second ones, as I rolled a 57, against the roll of 60…
I know Blizzard won’t make this right, but I still want to bring attention to it… and well… vent…

They have not acknowledged this is an issue. There have been many complaints. Restrictions on rolling need and winning duplicate items are not working as they said they were going to.
The only problem I see here is MMO’s relying on people to make them enjoyable.
“Community” is so 2006.
I’m 99.9% sure it’s because their “check” to make sure someone can’t get 2 of the same item is to double check the inventory of the person when the second roll goes out for the item.
But since you have to create the tier piece, it doesn’t register as the person owning the tier gem, and thus they’re eligible for it again.
They really just need to go back to Personal Loot, it’s honestly not worth the buggy Group Loot. Even if they got rid of all of the bugs, Personal Loot would still be a better system.
Just make some adjustments to Personal Loot, like removing the trading restrictions and add an ability for us to put an item we win up for roll to eligible people.

I don’t mind Need before Greed… it speeds up the dredging process of loot in general…
If I could have a suggestion, is to SLIGHTLY rework the system… something like this…
5 pieces of loot drop… but not all 5 pieces are rolled on at first. You can see ALL the drops, but you cannot roll on them all at once, first piece automatically comes up, you have 15 seconds to roll, after that, it goes to the highest roller… and then you continue the same process one items 2, 3… etc… THEN, the rule that allows seeing what is in people’s inventory would work flawlessly…
With this system, it makes it very fair across the board and there is no headaches…
Oh, and on the note of … “What if 15 seconds isn’t long enough??”… you literally just killed the boss, you should know if you want any of that loot or not. That is my honest opinion…
afaik this is ‘part’ of the problem. but not quite the whole picture.
since tier gear can proc with sockets or tertiary stats (at least I think it can, I haven’t been able to find any definitive information confirming one way or the other.), but it won’t actually proc them until you turn in the token it ‘might’ still be an upgrade for you unless you already have a socketed piece.
since they let you roll need on a piece that would be an upgrade due to socket or tertiary stat, and the token ‘might’ be an upgrade according to those criteria, you can roll on it even if you already have it.
I get why its annoying right now, since its one of the only ‘reliable’ sources for tier sets. but in a couple weeks once we start getting catalysts for tier, people will be kitted out with their full set pretty quickly anyway. and after that, the only thing the tokens will be useful for is people fishing for procs so it does make some weird sense why it is the way it is. (even if I agree that its not great in the short term, imo they should have placed a temporary block on rolling for duplicate tier tokens at least until the catalysts are out.)
If this is true then their coders are hacks. Loot drops all have an item number specific to that type of loot. For example, a Mystic Garnet Forgestone is “196597”. There is no excuse for not tracking this during the roll to insure it only goes once to a player. To check the players inventory for duplicates seems nonsensical to me - what if the player won that type of loot on a different run but had it enchanted for a different spec? Would that make them ineligible?

I suggest they bring back Master Looter, or at least some version of it. I like how they had it back then, where it was the HR/SR system, and you had to wait until the end of the run to roll on gear.
ML though comes at a risk, hoping that the Raid Lead doesn’t just give it to their friends or doesn’t honor the rolls. But, if you have a reliable and dependable RL, it’s a great system.
Completely agreed, personal loot was the fairest and fastest loot system ActiBlizz has had so far. And yes, removing the trading restrictions would make it better.

It can happen with non-tier items as well.
Blizz had multiple options for making improvements to previous loot systems and instead they went with a straight up downgrade. Seems about par for the course with them.
just happened to me in LFR on 2 bosses.
the same item (non-tier) dropped twice. rolls went out, same person won both and looted both. now they have 2 of the same item.
