My group killed Don Carlos’ in Heroic Old Hillsbrad and the rare version of his hat did not drop. We did have a Beast Master hunt, but checked to make sure the killing blow was not his pet (it was a consecration tick from our tank paladin). Was told to submit a Bug by GM Ritorack

The hat he drops is a quest item, you have to have the quest from Old Carlos in Tanaris before you kill him in OHB, also it can be done on normal OHB as well.
The GM response indicates you are wrong. Theyre the one that told me to submit this bug. There is no heroic quest to pick up. This is the rare version of the hat that summons a wolf. Not the White quest item. Thank you for trying to help, but read the whole post first in the future.

Then do you mean the one that summons the spirit? that’s listed as a 68% drop rate, maybe it just didn’t drop for you, afaik the only 100% drop rate hat he has is the quest item.
EDIT: and before you say “the GM said” you should also know a GM told a player he couldn’t wear the rank 14 gear after he dropped a rank because “the item says on it require rank 14” so they’re not always the most competent of folks.
I met the same problem.
Back in 2008, players reported Don Carlos’ Famous Hat (rare quality) was 100% drop rate, in patch 2.4.1 (according to every wowhead comment of the item)
But I haven’t seen the hat two runs in a row. Nothing dropped after we killed Don Carlos in heroic OHB. A GM suggested me to report the bug here too.
