I finished the xal’atath questlines and got a buff called the Gift of Nzoth that gives you this sort of eyeball. It says I can cleanse this curse and you get the twitching eyeball toy. However. Is there any downside to removing it? Given that there is a toy for this.
best to keep it, nzoth will return one day, and those that have kept it on for this long will be rewarded with a cool line of dialouge or something
You hear some whispers and get some extra dialogue in the great seal, but that’s about it. I’ve had it ever since it first came out and nothing cool has been done with it.
It gives you some special dialogue from certain NPCs and you hear occasional spooky whispers, just flavor stuff. Otherwise it does nothing but decorate your forehead.
Only people who have the gift of nzoth buff can see the eyeball on others who still have the buff. If you choose to get rid of the buff, you’ll get a toy so you can get it back as a cosmetic appearance.
Leaning towards removing it so far.
The gift of N’zoth could have been so much more but sadly it is lacking.

It’s pointless.
In warmode you get a buff to kill 10 players of the same faction. If completed you get the title Servant of N’zoth and a toy. The toy gives you a 1 hour buff to kill players from both factions, also makes you neutral and friendly with enemy npc that will help you.
Don’t need the gift to get that quest.

Also, the eye disappears outside of BfA content. So you get reprieve from the eye, OR you are forced to keep on top of your transmog, depending how you look at it.
The only gift worth mentioning is the gift of that horrible hedgehog’s demise
