Yes. My priest had the Silent Star cloak in her Vault just last week.
No, vault looks at what you have and gives the exact same thing again.

Yeah, that cloak was the first unique thing I’ve seen in a while. Just so happened to be a week I only did LFR so it was unusable anyways.

Very rare items have a roughly 1/3 chance of a regular item to appear in vault and a 8/10 Chance to appear over a regular item on a boss kill.
I get that it would be a bit too OP if the Vault only gave you items you don’t have yet, but they really should consider reducing the chance of the Vault giving you an item you already have equipped.
They should also considering making it so that an item’s chance of appearing in the Vault gradually increases after every week until it finally appears. It feels lame that I haven’t seen the Voice of the Silent Star at all in my Vault despite doing a full Heroic clear every week

The boss kills can be corroborated by drop data on Wowhead. For example the boss Neltharion in the current Aberrus Raid, the Ashkandur has an estimated 29% drop rate while the “very rare” Nelthario’s Call to Dominance has an estimated 22% drop rate. 29/22 is about 1.32 or a +32% chance for a regular item over a very rare item. 8/10 = +80%, which seems wrong for the chance of a regular item over a very rare item based on drops reported to/collected by Wowhead.
What source did you use for very rare items appearing in the Great Vault? I would have assumed those are the same distribution table as for boss kills across the entire raid (assuming you killed Sarkareth and the entire loot table is in play).
Obviously, only killing the first three bosses limits the table to just those bosses, so one fishing for an Elementium Pocket Anvil from the vault is best served by only killing the first 3 bosses.
I use tha vault for tokens to exchange for gear sockets… I have not kept a single item that I can recall.

But if there are 28 “N” items +3 “VR” items the total pool is 31, and the chance of drawing an N item is 28/31 = 90.3% while drawing a VR item is 3/31 = 9.7%, and that’s assuming an equal weighting among all items. However, their assertion is that the the VR items are weighted by 0.4 probability less than the weighting for N items.
The calculation I did for 28 items with a 3.43% drop rate and three items with a 1.29% drop rate sum up to 100%. If we use Wowhead’s numbers of 3.40% and 1.36% it sums to 99.28%, which comes up short (they made a mistake somewhere).
1.36% vs 1.29% isn’t much of a difference, but the probabilities for a table of items should always sum to 100.0%, it’s not fuzzy.
Anyway, saying 8/10 chance chance for a VR over a N on a boss kill seems in error (more like +32% vs +80%). The 1/3 chance for the vault is “close enough” (should be 37.6% rather than 33.3%).

Well i got that storm chicken bow from vault of incarnates (lfr difficulty one) for doing lfr bosses and collecting the weekly vault. I hear that is a more rare drop for that time… but that is the only time i ever got something cool from vault so… lol

Facts. I do Kaz, Echo, and Sark in heroic Aberrus on my Evoker every week (I JUST WANT THE GEM 
0/10 kills so far since BLP) and last week I got Call to Chaos which I already have and this week I got OCE, which I also have. Lmao.
