Idk why I’m entertaining this but, the visage form is a magical construct, not one made of actual flesh and bone, damage to the construct transfers over to the original form cause its damage to the essence of the being, but anything such as zits or other things that aren’t damage to the essence are up to the creator of the construct to decide.
If you play D&D or other similar ttrpgs its kinda like the Alter Self spell.

Even though lizards don’t have sweat, they still have pores that could get clogged up.

I would assume dragons if they had pores could get zits.
