We don’t know who built the Dark Portal on Azeroth since Medivh was the only one who knew about Draenor. It’s possible that some arcane elementals, arcane golems, or some magical servants of his built it on Azeroth. The Dark Portal on Draenor was constructed by orcs though after they received a vision from Medivh at their most desperate time.
Medivh had both the power of the Guardian and that of Sargeras in him. He probably just conjured them. The Portal on Draenor was doing the heavy lifting.
I always assumed that Medivh built the Azeroth side portal in secret.
The orcs did. They built the one on Draenor using plans medivh told them, and once over they built the frame around the Azeroth side in the same manner. chronicles page 116
They did a rush job on building it considering it’s so small compared to the Outland Portal though that may just be how it was rebuilt by the Legion after Tides of Darkness for Burning Crusade.

Yes, no one else knew. In the Last Guardian book, while Khadgar is under the apprenticeship of Medivh, the guardian leaves Karazhan frequently to go to the Black Morass. I feel like he may have conjured some servants for the labor.
