Medivh’s curse was supposed to have aged him from a youth to an old man, and its been 20-30 years since then, either he is extremely healthy to look the way he does, or he reversed the curse somewhat.
He was reliving the battle in Karazhan, and experiencing again that horrible wrenching sensation as Medivh drew from him his magic and his youth. The magic had returned—indeed, in many ways it was far stronger now than before—but his youth was gone, torn from him long before its time. He was an old man now, at least in appearance. He still felt hale and hearty, and had as much endurance and strength and agility as ever, but his face was lined, his eyes deep-set, and his hair and fledgling beard a stark white. Though only nineteen, Khadgar knew he looked three times that and more.
From the book Tides of Darkness. He has the appearance of an old man but the health of a ~46 year old (he was born 14 years before the opening of the Dark Portal and as the above text says was 19 when he was cursed).
Shaving the beard off just makes him look younger.

So basically there was no real downside to the curse aside from vanity? If his inside is still young he’ll just be very saggy when he dies.

Well, he got drained heavily when the curse first took hold of him. But otherwise, it does seem like all his physical and magical abilities returned over time.

No, they confirmed he’s not de-aged. Just freshly shaved.
I’d imagine extended time with A’dal proved beneficial to his health and livelihood. Especially one that’s not as fanatical as others.

In otherwords the draining of Magic left him with longer Stress Lines(they were smaller but still there according to Chronicle), White Hair of Stress and Sunken Eyes(in comparison to his original eyes which looked a bit puffy according to Chronicle) of one leeched of nutrients.
He pretty much went through the same thing the Nightfallen go through though never recovered the damage(not that there was much damage).
The fact that Sargeras removed the bags under Khadgar’s eyes should be a plus for that guy regardless of more noticeable lines on his face(barely noticeable on his body I would say)!
The Novels say he looks like someone 19 years old times 3(57 that would be) and more. He looked like someone in his late 50s not his 60s!

Medivh may have cursed Khadgar with old age but in the end Khadgar ended up getting the last laugh. Khadgar got his staff, his tower, women of all races swoon over him since he had a shave, and he has the best dad jokes. Khadgar - 1 Medivh - 0.
It wasn’t a curse, it was Medivh draining his life to age him the same way Old Horde warlocks aged up orc children during their war with the draenei.
Khadgar just shaved.
