Death Grip player interactions

Death Grip, especially recently, has been completely failing for no reasonably discernable reason, specifically on players with no adverse debuffs on them. There’s been a long time known interaction involving elevations relative to the caster, that sometimes cause it to fizzle but recently, even on targets with no debuffs on flat ground, it feels like even every third cast of death grip is just failing, and using the cooldown.
Is there anything in the works to fix this?
