Only happened on one of my alts. No matter which difficulty I picked for the Dance Dance Darkmoon quest, it would put me in the easiest one almost every time. Tried it 8 times - 6 times picked the hardest difficulty and it started up the easiest; picked middle difficulty twice and got it correctly one time and the easiest difficulty the second time.
