A group I was with completed the H+ CoS dungeon today and several of us were not able to loot the item from the chest and are out the 2 badges for completion. We were not in a raid, no one left or joined. I tried logging out, reloading, running out of the instance and back in and was still no able to loot.

Were you not able to click on the chest at all? Or simply wouldn’t let you loot it while you could see it in the loot box?
If you couldn’t open the chest, someone could have been trolling you, as only 1 person can have the chest open at any given time.
Just my guess. Haven’t encountered this problem myself so idk.
This happened in HoL for my group. The quest item simply wasn’t on the boss.
