If you get turned into a zombie in CoH titan rune, when you’re ressed your Maelstrom Weapon will no longer proc at all. I asked in the Shaman forums and apparently a similar bug was happening during the scourge invasion event, and it has rematerialized with the CoH Titan-rune suffix.
Logging in and out fixes it, but especially since CoH is timed… that’s not particularly ideal.
