I completed Nightfall Sanctum on Tier 8 at about 645am this morning (before reset), was awarded my 603 gear from the bountiful chest. My Great Vault rewards didn’t update to reflect this. I waited until after reset and checked them again, to see if that would trigger an update. No luck. I just ran a Heroic to populate the first item on that row to see if that update would trigger anything, and still no luck.
It was common in Dragonflight for the first heroic or timewalking I did to not count toward the total, but this is a bit of a different deal. That T8 was a considerably larger investment in time and energy than a heroic. I’d rather it not be wasted.

I just experienced the same thing. I completed Fungal Grotto @ Tier 8, but nothing was updated. Just before that, I completed a Tier 7 - the last slot did not change from Tier 6 to Tier 7. As it stands, Great Vault rewards appear as Tier 7 / Tier 7 / Tier 6. What’s going on?
Mine are in the exact same state, 7/7/6. Then did an 8 and no change.
so… I figured out that you have to do 2 before it’ll change the first slot to the next highest number. So come Tuesday, assuming you do all 4 Tier 8 (bountiful) you should be at 8 | 8 | 7 (unless you do more Tier 8). That’s where I’m at presently.

Ahh, I just educated myself a bit more on the Vault. It looks like it isn’t bugged, it’s just a matter of the wording on the vault items being a bit misleading. Saying “complete A tier 8 delve” to increase the reward item level is not really accurate.
It’s more like “complete tier 8 delves until your 8th highest delve completed was a tier 8” in practice. Foolish of me to expect accurate information from in game tooltips.
Moderators can delete this, and my apologies to any dev who saw it and rolled their eyes. I haven’t played with the vault system with varying difficulty levels up until now.
nah man it is messed up, I did like 15 t8’S This week, my rewards are at 8/8/1. The last slot says do a tier 2 or higher delve to increase this reward… so I kept doing 8’s over and over around 4 per day, all the bountifuls ya know? still no update. it sucks being stuck with a 584 slot in the vault when I’m at 603+ on every slot. Why bother doing more delves If I can’t earn another option for upgrade?
The issue persists. I ran 4 Tier 8 Delves tonight, but it only registered 2 of them in the GV list.
This is not bugged. 616 Great Vault has never deviated from the following requirements:
2x T8 Delves = 1x 616.
4x T8 Delves = 2x 616.
8x T8 Delves = 3x 616.
No amount of world quests or activities other than T8+ delves will increase the ilvl. It is strictly 8 T8 delves for the maximum number of 616 slots.
