Click casting doesn’t work on unit frames (target/party/raid, etc) if they use a modifier (alt, shift, control…). Right and Left mouse click work as intended, but with any modifier, it will cast on the player instead of the proper mouseover.
Example: I have Power Word: Shield bound to my right click and Renew bound to Shift+Right Click. My mouse is over “Johnny Awesome”'s frame on the party frame.
If I Right click. It will cast Power Word: Shield on Johnny Awesome.
If I Shift+Right Click, it will cast Renew on myself.
Tried with all addons removed, tried with the Clique addon, all behave the same.
Source: Many frustrating dungeons dispelling myself instead of the tank.
I am experiencing this too. I am amazed that you didn’t get any replies to this. Have you managed to resolve the issue?

I believe this is the conflict with the self modifier. At least it was for me. I turned self cast off an on. Clique is what told me the issue. Said that the ALT key was a self modifier so when I disabled it and reenabled AUTO under self cast, it cleared up the issue somehow. Then I was able to cast ALT LEFT click on another person. Hope that helps.

Bloodgreiva’s tip was right on. I’ve been having the same problem. When I used Clique with a mouseover or even targeting the player, sometimes the cast would cast on myself not the intended player.
Here is how to turn off self-cast modifier: Options, Combat, Scroll down to Self Cast, change the dropdown to None.
After I changed this, all Clique commands worked on the intended target
