By Expire does it mean if a target is killed this will still go off or just when it runs out? Thanks

Not sure about this one. There is a similar Anima Power in Torghast and it heals when the enemy dies or the DoT ends.
The legendary was nerfed during beta to only proc when the dot ends and not when the add dies with the dot.

That sounds pretty useless then since you’re mechanically encouraged to refresh DoTs rather than let them expire.
Yep, it was quite op in Beta so they nerfed it to this crappy version 
when the DoT has less than 30% time remaining, and you either refresh the DoT or kill the mob, the heal releases. It’s a 20 second debuff: if the enemy dies with 5.9 seconds of the timer left, you’ll be healed.
