We killed 10 man heroic sinestra for the first time. We killed it the same time that we died. A loot chest spawned, however it was unlootable. Boss didn’t respawn and we got saved to a 5/5 cleared raid. We also did not receive an achievement.

Our raid had this happen too. The chest spawned in somewhere during 1% while she was still alive.

Same thing just happened to my guild on recleaer, with people having not killed it before not getting their achievement.
