I get the message “item cannot be modified” even though I meet all requirements.
Same here, but not for every item. I noticed there’s another thread about the same thing only PvP gear.

I am having the same issue on several toons. I was able to upgrade a couple but most I cannot upgrade. This is Korthia gear mostly trying to go from 3/6 to 4/6.

I have been hearing about this going on with Korthia, Mythic+, and PvP gear saying they can’t be modified when trying to upgrade them. The developers are checking into this as a bug, thus a bug report would be best.
I’m not sure if these are all related yet the reports seemed to start around the same time so that may be the case. Keep an eye out for patches and hotfixes for when the cause is addressed.
