Hi, i can’t start “Threads of Fate” on my mage even though everything needed for “Threads of Fate” is done (main character monk, covenant campaign 3/9, achivments The Path to Ascension, Blade of the Primus, Awaken, Ardenweald, and The Master of Revendreth is done). Fatescribe Roh-Tahl gives no choice for “Threads of Fate” Please, help me.
I think ToF option is only offered to alts after a main has grinded through all the zones to 60 and joined a Covenant.

To activate the Threads of Fate system, you must first complete the Shadowlands Introduction Questline in The Maw, or complete the Maw skip if this option is open to you. Once you successfully complete The Maw and arrive in Oribos, Fatescribe Roh-Tahl will stop time and approach you, offering the quest 
The Threads of Fate. At this point, you must choose whether you want to level by following the Shadowlands storyline or by using the Threads of Fate system.

That’s right, I skipped entering the maw, but I wasn’t given a choice for storyline or Threads of Fate. The storyline just started with no choice
Did you log out or disconnect when entering Oribos, prior to selecting your choice? I think it will automatically choose the campaign for you in this case. It is not a problem either way, as you can easily switch from the campaign to threads of fate by going to Roh-Tahl’s permanent location in Oribos, completely opposite on the map of where you meet him upon entering the city (see the coordinates in Moonphyre’s link above). Once you speak to him, he should give you the option to switch. Switching to threads is a permanent choice.
