You can still get it by winning the highmaul colliseum scenario that you can que from your garrison with the gladiators sanctum building.
To get it you have to be the last person standing after everyone else has died.

can you do it as max level or do you have to be a certain other level?
You can do it at max.
The hard part is getting enough people to que for the colliseum as its pretty dead in the current expansion.

It requires at minimum 10 people be queued for it.
Theoretically still possible, but functionally will be pretty difficult finding that many people to both fulfill the outdated Garrison requirements and simultaneously queue for it.
Level 3 Gladiator Sanctum would be quite the unpleasant farm in Ashran nowadays.

Best bet is probably coordinating with people in a community or collector / secrets finding discord to farm it together.
