How would u be able to get that as a twink? I keep seeing it on twink guides…
Doing the quest gives you an item. That item is then right clicked and added to Armour piece.
This can also be done from the trade window. So you can do the quest and sell it to someone else.

The item is BoP, you can sell the click.
They put twink hat in the will not be traded part of trade window. You right click Arcanum and cast it item. Then trade.
