So I noticed that if you have the quest “Fall of the Betrayer” quest you must have a the medallion of Karabor you receive earlier - now normally one cannot destory or delete quest items so quickly especially if another quest is contingent on it and if you do there is a in game system to retrieve the item - there is no such in game system for the medallion and no way to restore it - It may be a old quest but a present Bug nonetheless
